Complaint Form Satisfaction Survey


The Office of the Information Commissioner of Canada (OIC) is currently looking for ways to improve and streamline the access to information registration complaint process.

To help us with these improvements, we would appreciate it if you would take part in the following survey measuring your level of satisfaction with the current complaint form. The survey should only take 3-4 minutes to complete.
